It's been a long time since I did anything like this (I used to use some of these kits doing lighting/audio system installs in theatres) so I'm out of touch with the tech and brands.

My TV is now mounted above a fireplace. Obviously, my components are not nearby. I'm looking for something to extend the IR so I can point the remote at the TV (intuitively) and change channels on the cable box, and adjust the volume on the receiver, and control the DVD player. Basic stuff. No dual-frequency HDTV stuff or anything like that, it all actually runs through my programmable Harman-Kardon AV receiver's remote; I just need to be able to point it in a different direction is all.

Any brands to go with? Stay away from? The other "issue" is, I need more than the normal 3' receiver cable (that fireplace being in the way makes for a long cable run). Any reason not to use a regular stereo 1/8" headphone extension wire, or will the signal degrade too much?

You can use a regular extension wire. It should work unless the run gets really ridiculous. You may want to consider a new remote control from a company called Universal. They have remotes that work both IR and RF. You could operate all of your equipment with the RF portion. The receiver for that system would be mounted with your other components. It will make the remote work basically anywhere in your house and won't require pointing it at anything. Only downside to this would be cost. The time required to program shouldn't be to bad unless you want to program tons of crazy macros for multiple control functions.
